In accordance with Article 13(1) and (2) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation), OJ L 2016, item 119.1 (hereinafter “Regulation”), we hereby inform you that:
1. the controller of your personal data is Mokotoska LLC with its registered office in Warsaw, ul. Mokotowska 58, 00-534 Warsaw;
2. the Controller has not appointed a data protection officer;
3. your personal data will be processed in connection with the Backstage Card discount program, i.e. pursuant to Article 6(1)(a) of the GDPR. a) GDPR and for marketing purposes, in particular for email contact and information about the services and products of the Controller and its partners.
4. The following data are processed: first and last name, business name, telephone number, email address, Tax Identification Number (NIP), and data necessary to determine Professional status in accordance with the requirements set forth in the BACKSTAGE CARD Program Terms and Conditions;
5. Your data may be transferred to marketing support service providers (companies handling text message and email sending);
6. Your personal data will not be transferred to a third country or an international organization;
7. Your data contained in the Application to Join the Program (the “Application”) is processed for the period necessary to process the Application. After its approval, only the data required for participation in the Program is further processed, i.e., first name, last name, email address and phone number, Tax Identification Number (NIP), and business name.
8. As an accepted Program Participant, your personal data will be stored for the period justified by the discount program or until you withdraw your participation in the Program or until you object to the processing of your data.
9. You have the right to access your data and the right to rectify, erase, restrict processing, the right to data portability, the right to object, and the right to withdraw consent at any time, without affecting the lawfulness of processing based on consent before its withdrawal.
10. You have the right to lodge a complaint with the President of the Personal Data Protection Office, address: ul. Stawki 2, 00-193 Warsaw, if you believe that the processing of your personal data violates the provisions of the Regulation.
11. Providing your personal data is voluntary and solely dependent on your consent. However, without your consent, you will not be able to participate in the discount program, and therefore we will not be able to fulfill our obligations arising from your membership.
12. Decisions regarding your personal data will not be made in an automated manner, and profiling will not be used.
