Attach at least 2 attachments to the application documenting your professional group affiliation, e.g.:

• portfolio (also in electronic form, including website address); portfolio documented in IMDb website – https://www.imdb.com/ or in the Film Polski website – http://www.filmpolski.pl does not require other documents) a ​​copy, transcript or scan of the entry in your National Business Register indicating  activities assigned to the Proffesionals – from the scopes listed in point 3.5,
  
• a link to the artistic agency representing the Professional,
• a certificate from the artistic agency representing the Professional,
• published sessions signed by the name,
• discount cards of other professional cosmetic brands,
• a school diploma in the field of the profession performed,
• a certificate from the school on the duration of education or a school ID, index (applies to students of make-up and beauty schools),
• a certificate confirming completion of a course/training organized by a professional make-up school,
• a link to the blog or channel on YouTube and/or Instagram,
• a reference letter confirming employment related to the performance of the work of the Professional (from the last 12 months from the date of submitting the application).
